Delivery Order Template for Singapore Businesses
A delivery order (DO) is the document that travels with goods and proves what was sent and received. This delivery order template Singapore guide gives you a copyable layout with every field you need, a worked example, notes on GST and the DO’s place among your other documents, and the mistakes that lead to disputes. Copy the tables below into your own template and adapt the header to your business.
What a delivery order is and why it matters
A delivery order is issued by the seller or sender and accompanies goods to the buyer. It lists what is being delivered and is signed on receipt, creating proof of delivery. It differs from an invoice, which requests payment, and from a purchase order, which requests goods. In practice the DO is your evidence that the right items reached the right place, which is why the receiver’s signature and date are the most important fields on it.
Fields every Singapore delivery order needs
- Header: your company name, address, UEN, and contact details, plus your logo.
- Document title and DO number: a clear “Delivery Order” label and a unique running number.
- Dates: DO date and delivery date.
- Deliver-to party: customer name, delivery address and contact person.
- References: customer purchase-order number and related invoice number, if any.
- Line items: item number, SKU/product code, description, quantity ordered, quantity delivered, unit of measure.
- Totals: total cartons/packages and total weight, useful for logistics.
- Remarks: partial delivery notes, handling instructions, or back-ordered items.
- Sign-off: issued-by, delivered-by (driver), and received-by with name, signature, company stamp and date.

Worked example
These details are illustrative. Suppose Acme Supplies Pte Ltd (UEN 201812345A) delivers to Bright Retail on 12/03/2026 against purchase order PO-5567. The DO is numbered DO-2026-0043. Line 1 is SKU BEV-OWN-COLA-320-06, “Cola 320ml 6-pack”, 50 ordered and 48 delivered; line 2 is SKU SNK-OWN-CHIP-100, “Potato chips 100g”, 100 ordered and 100 delivered. The remarks note “2 six-packs short-shipped, balance to follow on next delivery.” Totals show 9 cartons and 61kg. Bright Retail’s storeman counts the goods, writes the two-unit shortfall against line 1, signs, stamps and dates the received-by box. Because the shortfall is recorded on the DO itself, the later invoice can be matched and adjusted without dispute.
Delivery order and GST in Singapore
A delivery order is not a tax invoice, so it does not by itself account for GST — that is the job of the tax invoice, which shows the GST amount and your GST registration number. It is common to state values on a DO as reference, but treat the tax invoice as the GST document. If you are unsure how GST applies to your transaction, check current guidance from the Inland Revenue Authority of Singapore (IRAS) or your accountant, as rates and rules change over time. Keep DOs and their matching invoices together for your records.
DO, invoice and packing slip: how they differ
| Document | Purpose | Shows prices? |
|---|---|---|
| Delivery order | Proof of what was delivered and received | Optional |
| Packing slip | Lists contents inside a package for the recipient to check | No |
| Tax invoice | Requests payment and accounts for GST | Yes, with GST |
Common mistakes to avoid
- No unique DO number. Without a running number you cannot trace or match documents later.
- Missing received-by signature. An unsigned DO is not proof of delivery and offers no protection in a dispute.
- Not recording short or partial deliveries. If quantity delivered is not captured against quantity ordered, invoices and stock records drift apart.
- Confusing the DO with a tax invoice. Relying on a DO for GST leaves your accounting incomplete.
- Illegible or unstamped copies. Faint carbon copies without a company stamp are easily challenged.
- No copy retained. Keep at least one signed copy for your records and audit trail.

Frequently asked questions
What is a delivery order in Singapore? It is a document issued by the sender that accompanies goods and is signed by the receiver as proof of delivery, listing what was sent and in what quantity.
Is a delivery order the same as an invoice? No. A DO proves delivery and usually omits prices; a tax invoice requests payment and accounts for GST. They are separate documents that you match together.
Does a delivery order need to show GST? Not necessarily — GST is accounted for on the tax invoice. A DO may show values for reference only. Check current IRAS guidance if unsure.
What is the most important field on a DO? The received-by signature, stamp and date, because they turn the document into proof that the goods arrived.
How should I number delivery orders? Use a unique running sequence, for example DO-2026-0001, so every DO can be traced and matched to its purchase order and invoice.
